Structured Family Caregiving Waiver

Supporting Loved Ones with Alzheimer’s or Dementia at Home

What Is the Structured Family Caregiving Waiver?

The Structured Family Caregiving Waiver (SFCW) is a Missouri Medicaid program for adults age 21 and older who have Alzheimer’s disease or a related dementia and would otherwise require care in a nursing facility.

The program allows an eligible participant to remain at home with a live-in primary caregiver who provides daily care, supervision and support.

To qualify, the participant must meet Missouri’s eligibility requirements, including nursing facility level of care and qualifying MO HealthNet coverage.

What Does a Structured Family Caregiver Do?

A structured family caregiver provides the everyday help and supervision that can become more important as Alzheimer’s or dementia progresses.

The goal is to provide consistent support at home from a caregiver who knows the participant, their routine and their needs.

Who Can Be a Caregiver

The primary caregiver may be an eligible family member, non-family member or legal guardian.

The caregiver must live full-time in the same home as the person receiving care and meet SFCW program requirements.

A substitute caregiver must also be identified so there is a plan for care when the primary caregiver is unavailable.

The primary caregiver may have another job as long as it does not interfere with caregiving responsibilities or affect the participant’s health or safety.

Who May Qualify for SFCW in Missouri?

The Structured Family Caregiving Waiver is specifically designed for Missouri residents with Alzheimer’s disease or a related dementia who need a nursing facility level of care but want to continue living at home.
